Currently Enrolled Families Forms
All forms must be signed and dated after July 1, 2026 and submitted by August 1. Please be aware that children with incomplete required forms will not be able to attend school.
Remini is the app Gan Ami uses for school communication. You can access all forms by logging into your Remini account. Click on the 3 horizontal lines on the top left corner of the home page and choose “Forms” from the dropdown menu. To properly submit forms, you must use a computer, not a phone. Complete, sign, save, print and upload forms in your Remini account. Please make sure you hit the “Submit” button or we will not receive it.
Please Note: Some forms must be filled out and signed by your pediatrician.

Here are forms to help you connect with our school. First, please complete the “School Inquiry Form” so we have basic information to maintain communications. In addition to completing this form, you can also email Paula Sayag, Director, pls@bethami.org, with any questions or to schedule a tour.
