KAYITZ VOLUNTEER FORMS
FINGERPRINTING:
Every Kayitz volunteer must be fingerprinted if you have not already done so for Kayitz, Machane TBA or Gan Ami. If you have already been fingerprinted, you do not need to do it again.
Make a fingerprinting appointment at one of the American Identity Solutions (AIS) locations on a day and time of your convenience here. The reason for fingerprinting is “Child Care”
Bring with you to the appointment:
- A form of government identification such as a driver’s license or passport. If you do not have either, then your birth certificate and a school ID with a photo are sufficient.
- Your Social Security number (the actual card is not needed)
- Print and hand in the “Invoice Form” below:
CHILD PROTECTIVE SERVICES (CPS) BACKGROUND CHECK:
You must complete a CPS background check if you have not previously done one for us. Please review instructions and upload the notarized form. This is not the same as Fingerprinting.
(Remini please include a link for them to upload the scan of their form)
Temple Beth Ami will receive the results of your fingerprinting and CPS background check, and they will be stored in your confidential staff file in the Machane TBA/Kayitz office.
SUMMER 2026 PAID STAFF FORMS
All forms must be submitted by Friday, May 22.

MINORS WORK PERMIT:
If you are paid staff under the age of 18, you must complete the Minors Work Permit application every year.
– For “Type of Business” select “Arts, Entertainment and Recreation”
-Click “submit” when complete.
FORMS OF ID:
If you have never been paid through payroll by Machane TBA, Kayitz, or Gan Ami before, please provide forms of ID for your I9. Read here about the forms of ID that are accepted, and scan and upload the front and back of your ID in color.
